The B-cell maturation antigen-targeted chimeric antigen receptor (CAR) T-cell therapy, idecabtagene vicleucel (ide-cel; bb2121), is approved for use in patients with multiple myeloma who have had multiple prior treatments and whose myeloma has relapsed or is no longer responding. This analysis examined the relationship between ide-cel exposure and response, focusing on key efficacy endpoints and safety events. Patients in the phase II KarMMa study (NCT03361748), 127 in total, provided exposure data for ide-cel, having been treated with 150, 300, or 450106 CAR+ T cells at the appropriate dosages. Calculations of key exposure metrics, including the area under the transgene level curve from zero to twenty-eight days and the highest transgene level, were performed using non-compartmental methods. To quantify observed ER trends, logistic regression models were evaluated, utilizing both linear and maximum response functions of exposure on the logit scale. These models were then modified by including statistically significant individual covariates in a stepwise regression procedure. The target doses exhibited substantial shared exposures. The response rates, both overall and complete, demonstrated ER relationships that were influenced by exposure levels, with higher exposures corresponding to higher response rates. Evaluations using models indicated that female sex and baseline serum monoclonal protein levels no greater than 10 grams per liter were predictive of a higher objective response rate and a higher complete response rate, respectively. Safety events involving cytokine release syndrome, necessitating tocilizumab or corticosteroid treatment, were observed in the context of ER relationships. The period from May 2009 through December 2018 witnessed a review of patient notes for urodynamics, using SP lines for data acquisition. In 2014, our practice was altered to permit urodynamic testing concurrently with SP line placement in selected cases. Videourodynamics procedures necessitate the insertion of two 5 Fr (mini Paed) SP lines under general anesthetic conditions for the patients. Urodynamic examinations were scheduled for two patient cohorts: those undergoing SP line insertion on the same day, and those having the urodynamics procedure more than a day afterward.
