This paper stems from international collaborative workshops, dedicated to service mapping and prioritizing research in Cambodia's mental health sector.

Immune checkpoint blockade, a subset of cancer immunotherapy, has brought about a new era in treatment options by re-activating the patient's immune response against cancer. Still, the effectiveness varies, and only a small segment of patients experience lasting anti-tumor responses. Therefore, the development of innovative strategies to enhance the success of immune checkpoint therapy is critically needed. The process of post-transcriptional modification, N6-methyladenosine (m6A), stands out for its efficiency and dynamic characteristics. It is engaged in various RNA-related tasks, including the splicing, transport, translation, and degradation of RNA molecules. The immune response is fundamentally regulated by m6A modification, as corroborated by compelling evidence. The conclusions derived from these findings could lay the groundwork for combining m6A modification strategies with immune checkpoint inhibitors for cancer treatment. Currently, we consolidate knowledge on m6A RNA modification, particularly detailing recent developments in understanding how m6A modification impacts immune checkpoint molecules. In light of m6A modification's essential function in anti-tumor immunity, we examine the clinical meaning of manipulating m6A modification to maximize the efficacy of immune checkpoint therapy for cancer.
N-acetylcysteine (NAC) has proved to be a significant antioxidant agent, commonly used in the treatment of a multitude of ailments. This study investigated the impact of NAC on SLE disease activity and subsequent outcomes.
This randomized, double-blind clinical trial encompassed 80 subjects with systemic lupus erythematosus (SLE), who were grouped into two arms. A group of 40 patients was treated with N-acetylcysteine (NAC) at 1800 mg daily, administered in three doses spaced eight hours apart for three months. The remaining 40 patients constituted the control group, receiving their standard of care. At the beginning of treatment and after the study period, the British Isles Lupus Assessment Group (BILAG) and SLE Disease Activity Index (SLEDAI) scores, coupled with laboratory tests, quantified disease activity and measurements.
Patients receiving NAC for three months experienced a statistically significant decrease in BILAG (P=0.0023) and SLEDAI (P=0.0034) scores, as determined by statistical analysis. The control group exhibited higher BILAG (P=0.0021) and SLEDAI (P=0.0030) scores compared to the NAC-receiving patients, as observed three months post-treatment. Following the treatment regimen, the NAC group experienced a substantial reduction in BILAG-assessed disease activity throughout multiple organ systems (P=0.0018). The impact was most pronounced in mucocutaneous (P=0.0003), neurological (P=0.0015), musculoskeletal (P=0.0048), cardiorespiratory (P=0.0047), renal (P=0.0025), and vascular (P=0.0048) disease characteristics. Comparing baseline and post-treatment CH50 levels in the NAC group, the analysis revealed a substantial and statistically significant rise (P=0.049). The study participants did not report any adverse events.

The existing grant review system does not incorporate the distinctive methods and priorities of Dissemination and Implementation Science (DIS). The INSPECT scoring system for evaluating DIS research proposals utilizes ten criteria, mirroring Proctor et al.'s ten key ingredients. To assess pilot DIS study proposals through our DIS Center, we describe the method of adapting INSPECT and integrating it with the NIH scoring system.
In order to encompass a wider range of DIS settings and ideas, INSPECT was adapted to explicitly consider dissemination and implementation methods, among other things. Utilizing both INSPECT and NIH criteria, five PhD-level researchers with DIS knowledge ranging from intermediate to advanced, reviewed seven grant applications. Scores for INSPECT range from 0 to 30, with scores above 0 indicating better performance. Conversely, NIH scores range from 1 to 9, where scores below 9 are desirable. Grant proposals were each reviewed by two distinct evaluators in a preliminary step, before a group meeting to discuss experiences, scrutinize the proposals through both criteria, and reach consensus on the final scoring. To obtain further insights regarding each scoring criterion, a follow-up survey was sent to grant reviewers.
In a review of the overall scores, the INSPECT scores averaged 13 to 24, significantly higher than the NIH scores, which were observed to fall between 2 and 5. Proposals focusing on effectiveness and pre-implementation, avoiding the scrutiny of implementation strategies, benefited from the broad scientific perspective of the NIH criteria.
